IMPORTANT HEALTH COVERAGE TAX DOCUMENTS

The 2024 Form 1095c are prepared and available upon request. The Form 1095c provides information about offers of coverage made to full-time employees. To request a copy of your Form 1095c or for further information about Form 1095c, you can reach out to the HR Advisor for Benefits via email at HRBenefits@outagamie.org; by phone at 920-832-1671 or by mailing a written request to Outagamie County Human Resources, Attn: Employee Benefits, 320 South Walnut Street, Appleton, WI 54911.

 

2024 Form 1095b, which provides the coverage information for anyone that was enrolled in the Outagamie County employee health plan in 2024, will be sent by US mail to the mailing address on file by 3/1/2025 and should be received by mid-March.  Should you need to request a replacement copy, please contact WCA Group Health Trust at 800-236-6885.

 

Please note: Forms 1095b and 1095c are no longer required in order to file your tax returns.
