The Aging and Disability Resource Center (ADRC) is the first place to go with disability questions. The ADRC provides the public with current, unbiased information on all aspects of life related to living with a disability. The ADRC is a friendly, welcoming place for individuals to receive:

• Information and assistance
• Benefit counseling
• Long-term care options and enrollment counseling
• Short-term case management
• Health promotion
• Dementia and caregiver support

The Aging and Disability Resource Center (ADRC) strives to provide individuals with accurate and timely information and assistance relating to community resources and services such as:

• Adaptive equipment & assistance with disability accommodations
• Advocacy services & support programs
• Assisted living and subsidized housing
• Day programs
• In-home health services
• In-home personal care
• Information on the guardianship process
• Supportive employment and other work programs
• Transportation

The ADRC team is also dedicated to assisting individuals through Youth Transition by assisting you or your child in making the transition from the school system as smooth as possible by coordinating with parents/guardians as well as educational and vocational groups, and help make the right connections to resources that promote independence and inclusion. 

If you or someone you know is a: Outagamie County resident ages 18-59 with disabilities, a young adult transitioning from the school system to adult services or… a concerned family member, friend, guardian, or caregiver of a Outagamie County resident with any of the needs above… the ADRC can help.
